Introduction
Welcome to the Healthy Arizona Worksites online assessment, brought to you by the
Arizona Department of Health Services. You can use this assessment to:
- Determine how well your worksite is doing in terms of promoting physical activity,
healthy eating and other healthy lifestyles; and/or
- Get ideas for ways to increase support for healthy practices within your worksite.
- We welcome use of this website and the Healthy Arizona Worksites Assessment by all Arizona worksites.

For most worksites, several individuals representing various aspects of the worksite
environment will be involved in gathering information and completing the form. Although
each worksite has only one valid username/password combination for log in, several
people can log in with that username/password (concurrently or at different times)
to enter data on their worksite's form or to review any changes made by other team
members.
Multiple user access is convenient, but it can also be risky because users can inadvertently
(or inaccurately) overwrite each other's work. The most recent entries for a given
section are the ones that will be saved. With some simple planning, however, problems
can be avoided. For example:
The coordinator for the worksite's Healthy Arizona Worksites Assessment might assign
one individual to complete one section of the form. If a team member has input on
a section that is not his/her data entry section, they forward it either to the
coordinator or the individual in charge of that section.
OR
Designate one person to enter all data. Although the work is distributed among all
team members, they use a
Printable PDF
of the form to complete their section(s)
and forward to the designated data entry team member.
These are just two suggestions; there may be other approaches that work best for you and your worksite.

How to Complete the online Healthy Arizona Worksites Assessment
Visit the
login page and
register to access the Assessment. Upon login, you'll be routed to the Scorecard Page which displays your current scores based on the information you've provided to date. Only those who have your login information and the ADHS Healthy Arizona Worksites Assessment administrative staff can login and view your worksite's Healthy Arizona Worksites Assessment.

Data Entry
Data entry is very flexible to allow you to organize your work any way you wish.
You can complete the five sections of the questionnaire and the questions within
each section in any sequence.
A data entry session can consist of taking less than a minute to answer one question
or it can take an hour or two if you are working on the bulk of the questionnaire.
It is completely up to you.
There are no limits to the number of data entry sessions you are allowed in which
to complete your Healthy Arizona Worksites Assessment.
You can change or edit your answers anytime. To change an answer that you've already
selected, all you need to do is select another option or provide different information.
Corrections and changes can be made within a single data entry session or at any
subsequent data entry session.
More than one person from your team can access the Healthy Arizona Worksites Assessment
and enter data. Two users can even enter data concurrently from different locations.
However, it is important to coordinate your team's efforts to make sure one team
member doesn't "undo" another person's work.
To start data entry, find the questionnaire section (Organizational Support, Health Education/Promotion, Tobacco, Physical Activity, Healthy Eating) for which you wish to enter data. You'll be routed to that section. Data entry consists of clicking on the appropriate answer(s).
If you wish to enter data in more than one section during a single data entry section, click the button on the bottom of the section to save your data before proceeding to the next section. Also save your work at the conclusion of any data entry session. If your data entry session is longer than five minutes, we also recommend that you save every five minutes or more frequently. This is to prevent data loss in the event of power outages or system interruptions.
Your most recent saved data will appear the next time you login.

Saving Your Work
To avoid losing any data entry, please save your work:

often, even if your data entry session is still in progress - at least every five minutes.

at the end of data entry for any section before proceeding to another page or section.

Flagging and Correcting Errors
Please be sure to review each section of the assessment to ensure that every question has been answered. If you have not completed one section before moving to another, it may be helpful to make a note on the print version to remind you that you need to return to that section and complete the answer.

Scoring
Section and total scores are calculated automatically each time you save. To see your current scores (by section and overall), simply click the Scorecard menu item. If you've entered no data in a particular section, your score for that section will be zero. You can check your score anytime you wish by viewing the Scorecard page (you must log in to access the page). The Scorecard page is also the first page you'll see after logging in.
After you've completed your Healthy Arizona Worksites Assessment, your score will be very similar to your final score and in many cases will be identical.

How to Finalize
Upon completion of the assessment click the Finalize Survey button at the top of the assessment screen. When you finalize your assessment, a series of automated edit checks will be performed to make sure you’ve provided complete answers to all questions. Errors and or incomplete answers will be flagged so that you can easily find the questions that need to be corrected. Immediately after successful electronic submission/finalization of your Healthy Arizona Worksite Assessment, you will get an on-screen confirmation.
The confirmation will tell you when you can expect your final feedback report.

Final Feedback Report
All worksites that complete and finalize their Healthy Arizona Worksites Assessment will receive a detailed formatted feedback report which provides information and feedback about your worksite and the ways in which it promotes employee health. It may take a few weeks between the time you finalize your Healthy Arizona Worksites Assessment and the time you receive your formatted report.
In the interim, the Scorecard section of the website (accessible to those who have registered; appears after logging in) provides your final score information.

Important Tips
- Be sure to coordinate your team's data collection efforts so that one team member doesn't write over another team member's entered data.
- Edit and change your answers as often as you like prior to final submission of your Healthy Arizona Worksites Assessment. You can edit/change answers during a data entry session and/or at subsequent data entry sessions.
