Why complete the Healthy Arizona Worksite Assessment?

A worksite that promotes good health is a great place for employees. By completing the Assessment, worksites receive feedback about the extent to which they make it easy for people to be active, eat healthy and adopt healthy lifestyles.

What Information do Worksites Need to Register?

We recommend that you secure support of management before completing the assessment. We also suggest that you form a worksite wellness committee to help you complete the assessment and to take action on areas of need that you discover while completing it. The below information needed to register your worksite is usually readily available from your Human Resources department:

• Total number of employees.
• Percent of employees who are:
  • Full-time
  • Salaried
  • Eligible for employer-paid health benefits
  • Union members
  • Manual laborers
  • 2nd or 3rd shift workers
  • Away from the worksite half or more of their hours (e.g., sales)
